整改还好，主要原因分析老师通不过，要么说关联性没有，要么就说分析的是现象不是原因，要么就是没找到根本原因

整改措施应该不难，主要是原因分析不对，老师不能通过，只要原因分析正确了，针对原因进行纠正。{:1_90:}他给我的范例分析到根本原因是“某某文件没有明确规定*****”，我也分析到这步，他说这还不是根本原因{:1_93:}不是坑我吗

程序文件规定了要制定空压机的维护保养规程，实际上没有建立，为什么？

可能1、操作人员不知道这条规定→培训后实施。
可能2、操作人员知道规定单没有实施→检查考核。
……
总体来说，主要原因是没有对此工作进行检查，所以未能发现此项工作未落实，导致形成不符合。

→建立某种检查制度，落实具体责任。同时还要进行培训，要求人员明确职责和工作要求。

关于应急计划（应急计划未包含常见自然灾害）：1.        Why
文件编制人员未将“常见自然灾害”纳入应急预案中。
2.        Why
未组织多部门小组对“应急计划”进行评审，造成风险遗漏。
3.        Why
《应急计划控制程序》中未明确对“应急计划”进行评审的要求。
4.        Why
Root cause:

老师回复：原因分析不对，未评审？未评审就做吗？
要求继续改，上个版本我分析根本原因是公司没有将“应急计划”相关内容培训提出要求，就是没有策划应急计划的相关内容培训，也没有通过，。
请教各位老师，还要怎么分析才对？
